Most causes are related to out of date software. It is crucial that you take steps to keep any software you have installed on your site (including applications like WordPress, Joomla, Drupal, phpBB etc.) up to date as vulnerabilities can occur in these packages from time to time. Remember, BEFORE upgrading your software ALWAYS take a backup of your Home Directory and MySQL databases.
